Insurance Information

We are participating with some insurance.

It is our clients' responsibility to confirm with insurance carriers whether reimbursement is available through insurance plans, and to submit documentation to insurance carriers.

Insurance Paperwork - Admin Fee $25.00

This fee covers preparation of one set of documents (typically an itemized receipt) for submission for insurance. There is an additional $10 fee for any requested revisions to completed documentation.
